For placing orders for the free SR-5™ Valve recall repair kit, Bendix part number K140496, please send an email to ordermanagement.SVCDRT@bendix.com containing the following information:

• Contact Name and Phone Number
• Quantity of kits required (1 kit repairs 1 recalled SR-5 Valve)
• Kit shipment delivery address (cannot ship to PO Boxes)

To file a Bendix® SR-5™ Valve recall claim, please obtain and complete the correct document from Bendix® SR-5™ Trailer Spring Brake Valve Recall Claim Form and email it to warranty@bendix.com.
